Training Topic 1: Common Applications



As you know, upon referring a child for placement to OCOK (either verbally or via IMPACT), the CPS worker must complete and provide the OCOK Intake Specialist certain placement information & supporting documents/forms related to the child, all of which are outlined in the operations manual.


The Common Application (2087 or 2087ex) is one of the main & most important forms that is completed and provided to the OCOK Intake Specialist at the time of a child placement and services referral to OCOK.
